Rollover/Transfer my previous 401(k) to Nationwide

To rollover your previous 401(k) into the Company's Nationwide plan you will need to reach out to your previous retirement plan letting them know this is what you want to do. The Plan will send you documents to complete to be returned to them. Attached is the Nationwide form you will need to complete.


You will need to complete the following on the Rollover/Transfer Form: 

Section 1 : Plan ID: 88266; Plan Name: Motor City Dental Partners, LLC

Section 2: Participant Information

Section 3: Rollover/Transformation

Section 4: Submission Information - You will need to provide this information to your previous Plan

Section 5: Rollover/Transfer Details 

Section 6: Read over, if applies provide information

Section 7: LEAVE BLANK, HR WILL HAVE THE PLAN SPONSOR COMPLETE 

Section 8: Sign and date


Once completed open a ticket, attach, and submit to Human Resources for the final signature and processing.
