Team member got injured while working, what do I do?

In the case that 911 does not need to be called:

  1. Notify your immediate Supervisor/Doctor
  2. Complete the Employee Incident Report, in the case of a needle/wire stick complete the attached documents. 
    1. Page 1 in full
    2. Page 2 Give to your immediate Supervisor/Doctor to complete the top portion. If he/she is not available that day proceed with next step. 
    3. Page 2 Sign bottom section
  3. Open a ticket in Freshdesk with the 2 Page document attached.
  4. Go to the nearest Urgent Care for medical treatment.
  5. If seek treatment submit all documents from medical provider via Freshdesk ticket. 


If the injury warrants 911 to be called:

  1. Call 911
  2. Notify Supervisor/Doctor of injury
  3. Supervisor/Doctor or witness can complete Employee Incident Report, injured team member can complete document after seeking medical treatment
